Monthly Monitoring Reports <br /> The Discharger shall submit to the Regional Board and Department of Toxic Substances Control <br /> monthly operation reports by the 15`h day of the following month. These operational reports shall <br /> contain a summary of the results of monitoring, including effluent and injection well flow rates, volume <br /> of treated water, pressure readings, and water levels, operation and maintenance activities for that <br /> month, and a summary of any shutdown and/or spill events that occur that month. <br /> B. Quarterly Monitoring Reports <br /> Quarterly reports shall be submitted to the Regional Board on the 1st day of the second month following <br /> the end of each calendar quarter(i.e., by 1 February, l May, l August, and 1 November). At a <br /> minimum, the reports shall include: <br /> 1. A description and discussion of the sampling event and results, including trends in the concentrations <br /> Of pollutants, how and when samples were collected, and whether the pollutant plume(s) is being <br /> captured; <br /> 2. A summary of operation and maintenance tasks performed, including inspections, problems <br /> encountered repairs, maintenance, and equipment replacement performed(as applicable); <br /> 3. The reasons for and duration of all interruptions in the operation of the remediation system, and <br /> actions planned or taken to correct and prevent interruptions, if applicable; <br /> 4. Results of influent data and effluent and extraction well monitoring. Data shall be presented in tabular <br /> format in such a manner as to clearly illustrate trends in concentrations for each well over time; <br /> 5. A comparison of monitoring data to the effluent limits and discharge specifications and an explanation <br /> of any violation of those requirements; <br /> 6. Copies of all laboratory analytical report(s) that includes laboratory quality assurance/quality control <br /> sampling data; <br /> 7. A calibration log verifying weekly calibration of any field monitoring instruments (e.g.,DO, pH, EC <br /> meters)within seven days prior to their use; and <br /> 8. Any proposed change in the extraction well network with justification for the change. . <br /> C. Annual Report <br /> An annual report shall be submitted to the Regional Board by 1 February of each year. This report shall <br /> contain an evaluation of the effectiveness and progress of the remediation, and may be submitted with the <br /> fourth quarter monitoring report.
