No "Emergency Coordinator" has been designated. Facility's contingency plan has listed an <br /> Incident Commander and Environmental Health & Safety Mgr but no "Emergency Coordinator" as listed. <br /> At all times, there must be one employee either on premises or on call with the responsibility for <br /> coordinating all emergency response & reporting activities. Emergency Coordinator must have authority <br /> to commit the resources needed to carry out Contingency Plan and be thoroughly familiar with facility, <br /> Contingency Plan, and location of all records. Correct by updating the Plan with designating someone as <br /> an "Emergency Coordinator". <br /> Submit to SJC EHD within 30 days (3/24114): <br /> 1.one copy of the completed "Return to Compliance Certification" form <br /> 2. a photographic evidence waste fluorescent tube corrections <br /> 3. a written corrective action statement how the each the violations were corrected <br /> Notes: <br /> - Reviewed weekly hazardous waste inspection, monthly aboveground tank inspections. <br /> - Reviewed Source Reduction Evaluation Review & Plan, dated 09/15/11 <br /> - SB 14 Progress Report on-line reporting form completed in 2010. 4 year reporting cycle, next due <br /> 2014. <br /> - ERM (Excluded Recycleable Material) used oil tank will be ceased in used. ERM tank was used to <br /> collect oil from lehr to filter used oil and introduce back into lehr to lubricate gear rollers. Used oil no <br /> longer exempted as "recyclable material". Tank has option to be 'Permanently closed' according to CFR <br /> 112.7 <br /> - Sulfuric acid tank by the water retention pond and one by the Marley tower, were closed out in June <br /> 2013. Another tank by the retention pond,half full, still remains until site no longer uses the reservoir. <br /> - Corporate's managers has their internal plans to implement closure. Furnace has shut down in Jan 29, <br /> 2014, 3 weeks for cool down. Several equipment are being dismantled and distributed to other plants. <br /> Contact SJC EHD when facility has completed closure, removal of all hazardous waste. <br /> --Any assistance with needing for closure performed under EHD will be done under a billable <br /> consultation. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION ATA IME AT EHD"S CURRENT HOURLY RATE. <br /> EHD Ins Received Title: <br /> r <br /> EHD 23-02-003 Rev 44/19/12 CONTINUATION FORM <br />
