7h 1 --4 z4 <br />- _ CORPURATIQI, ;I <br />August 8, 2018 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue <br />Stockton, CA 95205-6232 <br />Attention: John Alaniz <br />Mr. Alaniz: <br />RECEIVED <br />AUG 13 2018 <br />ENVIRONMENTAL HEALTH <br />PERMIT/SERVICES <br />via registered mail <br />Enclosed is the Return to Compliance Certification and supporting documentation relating to the <br />Hazardous Waste Inspection conducted on 07/11/2018. <br />The supporting documentation enclosed addresses the following Items: <br />114: The site has spoken to the hazardous waste hauler and moving forward no manifests will be <br />accepted or signed without the required information. <br />601: The blue steel drum referenced in the inspection report was picked up by an authorized hazardous <br />waste hauler on 08/03/2018. The hazardous waste manifest is attached. <br />710: An operating procedure has been written and all team members authorized to use the aerosol <br />puncture station have been trained to this procedure. A copy of the operating procedure and proof of <br />training are attached. <br />711: Attached is the notification for aerosol treatment. It is my understanding that by submitting this <br />form in this packet addresses the notification requirement. If we need to send a copy to an additional <br />department in the EHD, please let us know. <br />According to an email received on 08/03/2018, supporting documentation has been received for Items <br />104, 109,110, 111 and 113 and these items have been confirmed as completed by the EHD.
