Attachment(A) <br /> Part(1) <br /> Spill/Release Response Plan <br /> 1. Reporting: <br /> a. Obligation To Report: The dealership is required to provide an immediate verbal report of <br /> any release or threatened release of a hazardous material that is harmful or potentially <br /> harmful to the environment or human health. A report is not required if it is reasonable to <br /> believe that the release or threatened release poses no significant present or potential hazard <br /> to human health and safety, property, or the environment. An example of a non-reportable <br /> release would be a small, say one-quart, spill of used oil on the shop floor. <br /> b. Reporting Authority: Any employee who detects a reportable release or threatened release <br /> of a hazardous material is to immediately notify his or her supervisor who shall then be <br /> responsible for immediately notifying one of the following parties who shall have reporting <br /> authority: <br /> • Brad Christian <br /> • Bob Hernandez <br /> If none of these parties can be notified then the supervisor has the authority to make the <br /> necessary report. Further, if the employee who detects the release or threatened release is <br /> unable to notify his or her supervisor or other dealership management, that employee is to <br /> make the required report. <br /> C. Reporting Procedure: This report is to be made to the San Joaquin County Office of <br /> Emergency Services at (209) 953-6200 and to the State Office of Emergency Services at <br /> (800) 852-7550 or(916) 262-1621. The report is to be made as soon as possible so long as it <br /> does not impede immediate control of the release or interfere with emergency medical <br /> measures and must include the following about the release or threatened release: <br /> • The exact location <br /> • The name of the person that made the report <br /> • The type and quantity of hazardous material(s)involved <br /> • The potential hazard(s)presented(known) <br /> Appropriate emergency response agencies and designated dealership managers are also to be <br /> notified. Emergency telephone numbers are listed on page A.1. <br /> d. Written Record: A written record of all verbal notifications is to be prepared by the <br /> individual who makes the notification and provided to the general manager.
