V. <br />r <br />September _, 1994 <br />5P <br />NIR JAMES G NEAL <br />LIKA CORPORATION <br />1443 NAVY DRIVE <br />STOCKTON CA 95206 <br />RE: REPORT OF VIOLATIONS AND SCHEDULE OF CORRECTIONS <br />On June 21, 1994 and June 22, 1994, San Joaquin County Public Health Services - <br />Environmental Health Division (PHS-EHD) conducted an inspection of LIKA Corporation <br />(LIKA) located at 1443 Navy Drive, Stockton, California. <br />As a result of that inspection, violations of the hazardous waste regulations were <br />identified. Specific violations and required corrective actions are listed below. Failure <br />to correct the identified violations within the scheduled time period will result in PHS- <br />EHD citing you for continuing/additional violations. <br />I. REPORT OF VIOLATIONS <br />1. Title 22, California Code of Regulations, Division 4.5, Section 66262.34 (f): <br />Plastic bags of hazardous waste did not have labels. <br />2. Title 22, California Code of Regulations, Division 4.5, Section 66265.173: <br />Contaminated debris was stored in a rolloff bin that was not covered. <br />II. SCHEDULE OF CORRECTIONS <br />Violation #1: Effective immediately, all containers storing hazardous waste shall <br />be labeled with the following information: <br />(a) The accumulation start date <br />(b) The words "Hazardous Waste" <br />(c) Composition and physical state of the waste <br />(d) Statement or statements which will call attention to the <br />particular hazardous properties of the waste <br />(e) Name and address of the person producing the waste <br />
