Specific violations and required corrective actions are listed below. Failure to <br />correct the identified violations within the scheduled time period will result in PHS-EHD <br />citing you for continuing /additional violations. <br />I. REPORT OF VIOLATIONS <br />Title 22, California Code of Regulations, Division 4.5, Section 66262.34 (f): <br />Drum containing hazardous waste was not labeled properly. <br />2. Title 22, California Code of Regulations, Division 4.5, Section 66265.16(d)(4): <br />Documentation verifying employee annual refresher training was not available. <br />3.
