• ' LFR, Inc. <br /> 6.0 PHYSICAL HAZARDS <br /> The following potential physical hazards may be encountered during scheduled <br /> activities at the Site: <br /> • slips, trips, and falls <br /> heavy equipment <br /> • heat stress <br /> • cold stress <br /> • noise <br /> • electrical sources <br /> • excavations <br /> • underground and overhead utilities <br /> • materials and equipment handling <br /> • confined space entry <br /> • biological hazards <br /> • elevated work platforms <br /> • fire/explosion <br /> • lightning/electrical storms <br /> • traffic <br /> 6.1 General Safe Work Practices <br /> • Workers will thoroughly clean their hands, faces, and other potentially <br /> contaminated areas before smoking, eating, or leaving the Site. <br /> • Respiratory devices may not be worn with beards or long sideburns, or under other <br /> conditions that prevent a proper seal. <br /> • Accidents and/or injuries associated with work at the Site will be immediately <br /> reported to the SSO. If necessary, an incident report will be initiated by the SSO. <br /> • Periodic safety briefings will be held to discuss current site conditions, field tasks <br /> being performed, planned modifications, and work concerns. <br /> • Site conditions may include uneven, unstable, or slippery work surfaces. <br /> Substantial care and personal observation is required on the part of each employee <br /> to prevent injuries from slips, trips, and falls. <br /> • Workers will maintain good housekeeping practices during field activities to <br /> maintain a safe working environment.
