LFR, Inc. • <br /> SIGNATURE PAGE <br /> The following signatures indicate that this Health and Safety Plan has been read and accepted <br /> by LFR personnel as well as subcontractors and their personnel. <br /> NAME COMPANY SIGNATURE DATE <br /> Important notice to subcontractor(s): <br /> This Health and Safety Plan has been prepared solely for the use of LFR personnel. It is <br /> supplied to you for informational purposes only and may not be relied upon for protection of <br /> your employees. The Subcontractor is responsible for providing, at its cost, all personal <br /> protective clothing and equipment required for its employees to perform their work in a safe <br /> manner and in compliance with all applicable state and federal OSHA regulations. <br /> Subcontractor is responsible for ensuring that such equipment is in good condition and is <br /> properly inspected and maintained. Subcontractor must, at a minimum, use the equipment and <br /> follow the procedures described in this HSP. Failure to do so may result in immediate <br /> termination of Subcontractor's services. This does not relieve Subcontractor of the <br /> responsibility to provide equipment and institute procedures affording a greater degree of <br /> protection than those specified in this HSP should Subcontractor determine such measures are <br /> necessary to protect the health and welfare of its employees, second-tier subcontractors, or <br /> others under its control or direction. <br /> Page 26 KMEP-ffiockmn-HASPvI.doc:LFR <br />
