Box 24447 <br /> Seattle, WA 98124-0447 <br /> RE: REPORT OF VIOLATIONS AND SCHEDULE OF CORRECTIONS <br /> On July 29, 1993,San Joaquin County Public Health Services/Environmental Health Division <br /> (PHS/EHD) conducted an inspection of Time Oil Company, located at 3015 Navy Dr., <br /> Stockton, CA. <br /> As a result of that inspection, a violation of the hazardous waste regulations was identified. <br /> The specific violation and required corrective action are listed below. Failure to correct the <br /> identified violation within the scheduled time period will result in PHS/EHD citing you for <br /> continuing/additional violations. <br /> I. REPORT OF VIOLATIONS <br /> 1. Title 22, California Code of Regulations, Division 4.5, Section 66265.16 (d). <br /> Personnel training records were incomplete. <br /> II. SCHEDULE OF CORRECTIONS <br /> Violation #1: Effective immediately, personnel training records shall contain all <br /> elements identified in referenced code section. <br /> Your letter dated August 9, 1994, stating that all violations have been corrected, has been <br /> received by this office. <br /> PHS/EHD may conduct a follow-up inspection at the facility to verify compliance. <br /> The issuance of this report of violation and schedule of correction does not preclude <br /> PHS/EHD from taking any administrative, civil or criminal action as a result of the violations <br /> noted herein. <br /> A Division of San Joaquin Couniv Health Care Services <br />
