Wright Contract No. 9120 <br /> Work Authorization No.2 0£ axly eN26"wedi <br /> Pursuant to paragraph I of the General Contracting and Consul ng Services Agreement <br /> ("Agreement) dated December 11, 2003 by and betwee Wright Environmental <br /> Services, Inc. ("Contractor"), and Universal Forest Products ("Client'), Client hereby <br /> authorizes Contractor to perform the following scope of work at its Thornton, California <br /> Facility subject to the following conditions set forth below: <br /> • Provide all the labor to obtain permits from the San Joaquin County <br /> Environmental Health District (SJCDEH) and the local Fire Department if <br /> applicable for the removal of (1) 10,000 Gallon Diesel Underground Storage <br /> Tank (UST); <br /> • Provide labor and equipment to vacuum remove and dispose of approximately <br /> 10,000 Gallons of Diesel contaminated water found within the existing UST; <br /> • Provide labor, equipment and materials to excavate and expose (1) 10,000 <br /> Gallon Diesel UST; <br /> • Provide labor, equipment and materials to inert (1) 10,000 Gallon Diesel UST <br /> with dry ice as required by SFCDEH; <br /> • Provide labor, equipment and materials to lift, load, transport and dispose/recycle <br /> of (1) 10,000 Gallon UST; <br /> • Provide labor,equipment and materials to take up to (3) soil samples and have <br /> them analytically tested at a State approved Analytical Laboratory for Total <br /> Petroleum Hydrocarbons as Diesel, BTEX, and Fuel Oxygenates; <br /> • Provide labor, equipment and materials to backfill tank excavation with clean <br /> imported Pea Gravel and Class II Aggregate Base materials; <br /> • Provide labor and materials to profile, manifest and provide the customer with <br /> copies of all documentation regarding the disposition of the contaminated liquid <br /> and UST. <br /> e. Letter report of UST removal. <br /> • Consult with client and advise on next steps should contaminated soil or <br /> groundwater be discovered during UST-removal task. (Professional Services will <br /> be provided at rates indicated on Standard Service Rates) <br /> Exclusions: <br /> • Shoring or bracing of any kind of the excavation area; <br /> • Remove, repair or relocation any existing utilities around or over the UST; <br /> • Removal, transportation or disposal of any encountered contaminated soils or <br /> groundwater; <br /> • Groundwater dewatering or stabilization of any kind; <br /> • Demolition or removal of concrete hold down pad that may be beneath UST; <br /> • Residual liquid greater than 10,000 gallons; <br /> • Compaction testing of fill material; <br /> • Plastic liner in excavation; (May be required if contaminated soil is encountered) <br /> • Site assessment work associated with a leaking underground UST. <br /> Page 1 of'2 <br />
