The owner/operator shall maintain the <br />following documents and records at the facility: <br />a) the job title for each position at the facility related to hazardous waste management, and the name of each employee filling <br />each job, <br />b) written job description for each position listed in a) above, <br />c) written description of the type and amount of both introductory and continuing training that will be given to each person <br />filing positions listed in a) above: <br />d) records that document that the training has been given to and completed by facility personnel. <br />Training records on current personnel shall be kept until closure of the facility. Training records on former employees shall be <br />kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial <br />or refresher hazardous waste training. Within 30 days (July 14, 2011) provide this office with a copy of the roster and the <br />syllabus which meet the requirements of 66265.16 CCR (California Code of Regulations). <br />60, 61, & 62. Manifest numbers 005920256JJK, 007124862JJK, 007431374JJK, 00510762�JJK, 005107630JJK, were <br />found without a signed off copy from the destination facility. Manifest numbers 005107626JJK, and 005107627JJK, were not <br />found on site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for <br />three years and have them readily available for review. If the generator did not receive a copy of the manifest with the <br />handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of <br />the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or <br />operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed <br />off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall <br />submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator <br />explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC.
