Jiffy Lube <br /> 730 South Beckman Road, Suite B. <br /> Lodi, Ca 95240 <br /> Dear Mr. Von Flue, <br /> This letter is in response to the Unified Program Hazardous Waste Inspection <br /> that was performed at our Jiffy Lube store located at 1130 North Main Street in Manteca <br /> on June 17,2004. During the inspection, we were sited for seven minor violations. <br /> Enclosed is physical proof of correction(photos, invoices etc.) for the following <br /> violations: <br /> #19-accumulation start date labeled on 55 gallon containers in dumpster area �D��r G o <br /> DN —SCTC <br /> #20- 55 gallon drums and one 5 gallon container labeled according to Title 22 CCR <br /> #21 -metal rungs on 55 gallon drums bolted and sealed- DRv«'l CEA v i5� <br /> #27 - invoice showing pickup and removal of fuel filters separate from oil filters <br /> #88a.-Hazardous Waste Emergency Information sheet filled out and posted on the wall <br /> next to the telephone in shop lobby <br /> #88b. - used crushed oil filters buckets labeled in compliance with CCR, sec. <br /> 66266.130c. (3) <br /> - 500 gallon waste oil tank properly capped and labeled <br /> - oil filter crusher run-off bucket properly capped and labeled as well as <br /> hose clamps secured to run-off spout <br /> In response to violation#12, which was corrected onsite in the presence of Mr. <br /> Von Flue,the employee responsible for the violation has been reprimanded as well as the <br /> other shop employees,that any hazardous waste spills shall be cleaned up in accordance <br /> with the policies and procedures outlined by the Environmental Health Department of <br /> San Joaquin County.
