0 0 <br />The following is an itemized list of hazardous waste violations that have not been <br />addressed for MARCH AND BIANCHI CHEVRON as of May 20, 2015. <br />Open violations from April 02, 2015 inspection <br />Violation #109 - Failed to keep signed copy of manifests from the designated facility for three years. <br />Copies of uniform manifests for the disposal of hazardous waste for April 2, 2012 through April 2, 2015 were not four <br />on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br />receipts used in a consolidated manifesting procedure on site for three years and have them readily available for <br />review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and <br />submit copies to the EHD. <br />Violation #112 - Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for April 2, 2012 through April 2, 2015 were not found on site. Hazardor <br />waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br />consolidated manifesting procedure on site for three years and have them readily available for review. Immediately <br />locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br />EHD. <br />Violation #501 - Stored hazardous waste onsite greater than 180 days. <br />Two 55 gallon containers of hazardous waste did not have a start accumulation date and therefore could not be <br />determined how long they were on site. The Manager Jeet Kaur said that they were last emptied sometime lastyear <br />and that both containers have waste inside of them. Facilities who generate less than 1000 kg of hazardous waste <br />per month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. <br />Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy o <br />the manifest to the EHD. <br />Violation #605 - Failed to completely label containers or tanks of hazardous waste. <br />Two 55 gallon containers of hazardous waste were not properly labeled. One drum that was located in the front of thr <br />dumpster area was missing all the information except the physical state and the words hazardous waste. The other <br />gallon container that was located in the back of the dumpster area was missing all the information except the words <br />hazardous waste.. All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste' <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required information. Since <br />it cannot be determined how long the two 55 gallon containers have been on site, immediately contact a licensed <br />hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br />Page 1 of 1 <br />
