Universal waste lamps shall be stored in a manner that <br />prevents breakage and release to the environment. All broken universal waste lamps and releases shall be <br />immediately cleaned and containerized. Immediately store all universal waste lamps in a manner that prevents <br />breakage and release to the environment and submit evidence of correction to EHD. This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by February 7, 2015. <br />This is a final hazardous waste inspection for the previous owner, Tom Migliori, who has sold the business, and a <br />Routine hazardous waste inspection for the new owner. Escrow was closed on approximately January 5, 2015. <br />Consent to perform the inspection, take photos and collect samples was given by Suleiman EI Billeh and Michel <br />Haddad, the new business owner. Tom Migliori was also on site during this inspection. <br />This facility generated less than 5 tons of waste in the last calendar year (Jan-Dec). <br />Verified all hazardous waste has been removed by Tom before handing the business over to the new owners. <br />During this inspection Tom provided a copy of the last hazardous waste disposal manifest. <br />Discussed with new owners hazardous waste (HW) and universal waste (UW) storage, labeling, disposal, and <br />record keeping requirement.
