TASK 2—PUBLIC NOTIFICATION <br /> GHD will provide assistance and information to complete the public notification process for site closure <br /> consideration. The proposed scope of work includes: <br /> 1. Obtaining available property owners names, addresses, and Assessor's Parcel Numbers for a radius of <br /> not less than 200 feet and not more than 300 feet from the boundaries of the site. <br /> 2. Prepare a list of properties and transmit list to the CVRWQCB. Mailing labels will be provided if requested <br /> by the CVRWQCB. <br /> 3. Prepare a Fact Sheet for the site to be distributed to the adjacent properties as required for public <br /> notification. <br /> 4. GHD will post the notification letter at the site. <br /> Deliverable: Property information from radius search for public notification and post the accepted public <br /> notification letter and Fact Sheet. <br /> TASK 3—WELL DESTRUCTIONS —WORKPLAN,PERMITTING, USA MARKUP,WELL DESTRUCTIONS,AND REPORTING <br /> Once case closure status is granted, GHD will destroy the site monitoring wells and document the field activities. <br /> Below is a list of the tasks required to complete this scope of work: <br /> 1. GHD will prepare and submit a Well Destruction Workplan describing the drill out of up to six(6) 2-inch <br /> PVC monitoring wells and place cement grout from the bottom of the boring to within two feet from the top <br /> of the borehole. The Workplan will include a HASP and will be submitted to CVRWQCB for approval. <br /> 2. Prepare a well destruction permit application and submit to the CVRWQCB to obtain property owner <br /> signatures. Submit signed permit applications and fees to the San Joaquin County Environmental Health <br /> Department(SJCEHD). <br /> 3. A site visit to mark the well locations and notify USA is required. No independent subsurface locator is <br /> necessary for the well destructions. <br /> 4. Notify the CVRWQCB, SJCEHD, and the property owner of the intended work and schedule. <br /> Deliverables: Well Destruction Workplan, well destruction permit applications, Well Destruction Report, <br /> State Well Driller's Reports, and upload data to GeoTracker. <br /> TASK 4—COORDINATION OF SOIL AND PURGE WATER DISPOSAL(3 EVENTS) <br /> Collect the required representative samples to profile and gain acceptance for the disposal of wastes generated <br /> during the groundwater monitoring activities and well destructions (2 events total). <br /> 1. Collect representative samples from the waste to characterize and complete the waste profiles for waste <br /> disposal/recycling. <br /> 2. Coordinate the off-haul and disposal of purge water generated from the quarterly monitoring events. <br /> 3. Coordinate with the licensed waste transportation company to remove and transport to the appropriate <br /> disposal/recycling facility. <br /> 4. Retain transportation Bill of Lading, landfill gate receipts, and recycling facility acceptance receipts and <br /> include in the subsequent associated report. <br /> 5. Waste will be removed after each of the following activities: <br /> • Well destructions. <br /> • Every other scheduled quarterly groundwater monitoring and sampling event(if needed). <br /> Deliverables: Waste disposal/recycling documentation included in the associated well destruction and <br /> monitoring and sampling reports.
