UST Tank Information forms for all three tanks are not current in CERS. - The tank form for the 87 tank incorrectly states the vent piping is double walled and that there are no vapor recovery lines. - The tank form for the 91 tank incorrectly states the vent piping is double walled and that there are no vapor recovery lines. - The tank form for the diesel tank incorrectly states the vent piping is double walled and that there are no vapor recovery lines. It also states that the contents are midgrade fuel instead of diesel. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The response plan is not current and/or not approved by the EHD. Section V of the response plan has not been completed. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on February 17, 2015 and a test report was submitted on April 6, 2015, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 126 HSC 25404(e)(4) Failed to report program data electronically. The UST Monitoring Site Plan was not submitted to the the California Electronic Reporting System (CERS). Beginning January 1, 2014, all businesses are required to submit all new (or any changes to existing) UST information, including: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator Written Agreement (if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance online to the CERS at http://cers.calepa.ca.gov.
