The following are Minor-Notice to <br /> Comply violations: #104, 109/110/111 <br /> 109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on 12/15/2011 before today, and was 2 months past due. These tests are required <br /> once every 12 months. Take all necessary precautions to ensure testing is performed in a timely <br /> manner. <br /> 104. Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility <br /> NOTES <br /> The referenced facility is under receivership of Kevin Singer appointed by Contra Costa County Superior <br /> Court on October 16, 2012. The facility is currently operated by Five Star Restaurant Consultants who <br /> was retained by Kevin Singer. <br /> In a discussion with Louise Dawson, it is not known if the original owner, Abraham Cyrus, would retain <br /> ownership of the facility and the tanks. Please update and submit to EHD the current UST facility and <br /> tank forms if the information on the forms are determined to be different. <br /> Technician on site did not have verification of his current Veeder Root and CPT manufacturer's training. <br /> Technician stated cerification of manufacturer's training will be emailed/submitted to EHD within one <br /> business day.
