The following items were required to be completed within 30 days of the close of <br />escrow [CA Health and Safety Code section 25284(b) & (c)]: <br />1. Create an account and obtain facility access to the facility on the California Environmental <br />Reporting System (CERS) by going to http://cers.calepa.ca.gov. Ensure that all required <br />uploads are completed in a timely manner. <br />2. Underground Storage Tanks — Facility Form — Provide all current information. Log into CERS, <br />update the required information, and submit for review by the EHD. (Note: Any change of <br />information must be updated in CERS within 30 days of the changes.) <br />3. Underground Storage Tanks — Tank Forms — Provide all current information in CERS. <br />Complete a separate tank form for each tank. <br />4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the forms <br />provided, and upload in CERS. This document is required to be submitted annually. <br />5. Underground Storage Tank Monitoring and Response Plans: Upload in CERS. Retain an <br />approved copy of these plans on site for future reference. <br />6. If you are the owner, you must submit a notification form identifying the "Designated UST <br />Operator" for each facility owned. A notification form is enclosed for your use. <br />7. Report #5021: Update all information for all programs at this site including the UST and <br />Hazardous Waste programs, sign and date. Bring with all other forms to our accounting office <br />to pay for transfer fees. <br />8. Go to: h_ttp://www.waterboards.ca.gov/cwphome/ust/regulatory/index.html#rgsto obtain the <br />current statutes (Underground Storage of Hazardous Substances) and UST Regulations that <br />you are required to have for your monitoring and response plan. <br />9. Enclosed for your information is a brochure of Best Management Practices (BMPs) for Retail <br />Gasoline Outlets that you can use as part of your BMPs for your facility. <br />1868 E.
