An accurate UST Monitoring Site Plan was not submitted. The monitoring site plan did not show the locations of fill sump sensors or annulars sensors and included an inaccurate location for the monitoring panel. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was performed on 8/9/16. A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. Missing records include: -2016 Monitoring System Certification test results -2015 Monitoring System Certification test results -Alarm logs /work orders in response to fuel alarms on 3/6/16 (1_12 diesel fill), 3/14/16 (1_8 87 STP), 2/4/17 (1_13 91 fill), 2/10/17 (1-13 91 fill), 2/10/17 (1_8 87 STP), and 2/20/17 (1_8 87 STP) These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site.
