Pescadero Avenue,Tracy, CA 95304 PERMIT/SERVICES <br /> EVACUATION PROCEDURES <br /> EMERGENCY COMMUNICATION ALERT <br /> Supervisor contacts terminal manager,communication alert chain and starts evacuation <br /> process. <br /> COMMUNICATION ALERT CHAIN <br /> In the event of an emergency first person at the incident should contact the proper <br /> authorites; the preferred means is to dial 9-911. <br /> PHONE TREE <br /> The phone tree is in the event of an evacuation to notify all employees as soon as <br /> possible. <br /> If the incident is in the garage the shop supervisor will be notified. <br /> -Kris Blankenship will notify Jack Schoell, Ken Nolan, and Harley Freitas. Open main <br /> truck gate for first responders to arrive and direct traffic until incident is over. <br /> -The employee that reported the incident to Kris will use the public address system to <br /> announce an evacuation. <br /> -Jack to notify linehaul office,drivers,evacuate to man location 2 (northeast corner of <br /> yard) and start roll call to account for all employees. <br /> -Ken will notify Lou(x1534), Kim(1531), Anna(x1531) and Tom Pereira(x1520).Help <br /> evacuate employees and man location 4 (main pedestrian gate) to start roll call and <br /> account for all employees. <br /> -Harley will notify all supervisors, SOM, tower 1, tower 2,help evacuate employees, <br /> meet and man location 3 to start roll call. <br /> -Lou to notify the dispatch office,drivers, evacuate employees, man location 1 (northwest <br /> comer of the yard, along USF fence and at the gate), and start roll call. <br /> -Supervisors will notify all dock employees and help evacuate to the safest locations at <br /> location 1 (northwest side, along USF fence and at the gate),location 2 <br /> (northeast corner of yard),location 3 (employee turnstile),and location 4 (main entrance <br /> pedestrian gate). <br /> -Kim will notify GOC/OS&D in dock office,Eric at USF, and meet at location 1 to help <br /> Ken with roll call. <br /> -Anna to notify facilities staff downstairs (x1520), upstairs W&M's (1515), sales (and <br /> meet at location 1 to help Ken with roll call. <br /> -Tom Pereira,Facilities, to meet with Ken Nolan at location 1 to assess damage. <br />
