SUMMARY OF CORRECTIONS <br /> #10 - Failed to determine hazardous waste: <br /> Sample has been taken and sent to lab for analysis (see <br /> sample pick up receipt.) Results were not available at time of <br /> report due to length of test. Results will be faxed to your office <br /> upon receipt. <br /> #15 - Decontamination equipment not provided: <br /> An eye wash station has been provided at the Hazardous <br /> Waste storage area (see picture #1 ) <br /> #20 — Failed to properly label Hazardous Waste: <br /> New labels have been placed on our Hazardous Waste <br /> containers and have been labeled properly (see pictures 2a <br /> and 2b.) <br /> #88 — Stored Hazardous Waste onsite for greater than 180 days: <br /> Old Hazardous Waste oil has been properly picked up and <br /> Disposed of ( see receipt of pick up titled picture 3.) <br /> #90 — Modified emergency information lacking or insufficient: <br /> An emergency coordinator has been assigned (see Diamond <br /> Pet Foods emergency preparedness plan.) He is to be notified <br /> in the event of any emergency and will coordinate response <br /> accordingly. Emergency phone lists have been developed as <br /> well as proper reporting procedures. <br /> #91 — An empty HW container was not marked with the date it was <br /> emptied / not managed within one year of being emptied. <br /> All drums are now disposed of properly through use of the <br /> manufacturer ( see pick up receipt.) <br /> Notes: Free CUPA class provided call and sign up for one: <br /> I called and have been signed up for the 4 hour class on May <br /> 27th and the 8 hour advanced class on June 24th per the <br /> investigator's suggestion. <br />
