SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 600 East Main Street, Stockton, California 95202-3029 <br /> Telephone:(209)468-3420 Fax.(209)468-3433 Web:www.sioov.orp/ehd/unitiii.htrnl <br /> CONTINUATION FORM Page: 3 of 3 <br /> OFFICIAL INSPECTION REPORT Date: 07/09/10 <br /> Facility Address: 25460 Schulte Rd., Tracy Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II or MINOR-Notice to Com <br /> ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION. <br /> Notice to Comply - Minor violations: <br /> 452 No emergency coordinator; No modified contingency plan, Failed to train all employees on waste <br /> handling and emergency procedures. <br /> No emergency coordinator was identified on site. A contingency plan could not be located on site; no <br /> modified contingency plan was posted. Per employees on site at time of inspection, facility does not <br /> conduct training in hazardous waste handling and emergency procedures. Employees were unfamiliar <br /> with hazardous waste requirements and spill handling. At all times there must be at least one employee <br /> either on the premises or on call (available to respond to an emergency by reaching the facility within a <br /> short period of time) with the responsibility for coordinating all emergency response measures, titled the <br /> Emergency Coordinator. The following information must be posted near a telephone: name & telephone <br /> of the emergency coordinator, location of all fire extinguishers, fire alarms & spill control equipment, the <br /> telephone number for the fire department. All employees shall be thoroughly familiar with proper <br /> hazardous waste handling and emergency procedures. Immediately designate an emergency <br /> coordinator for this facility, post the required emergency information (modified contingency plan) near a <br /> telephone, and train employees in all necessary hazardous waste handling and emergency procedures. <br /> Submit a copy of the posted modified contingency plan to the EHD within 30 days (8/9/10). <br /> Complete & submit a copy of the Return to Compliance Certification to the EHD within 30 days (8/9/10) <br /> along with any supporting paperwork and a statement documenting the corrective actions that have <br /> been or will be taken for each violation. <br /> Notes: <br /> - Per assistant manager, facility opened in November 2009 and no hazardous waste has been <br /> generated at this facility since that time. Absorbent and waste containers are available on site, however, <br /> spent absorbent from spill cleanup may not be handled properly. Ensure that all hazardous waste <br /> contaminated absorbent is being handled properly as a hazardous waste. <br /> - Disposal receipts for all hazardous wastes must be maintained on site for at least three years, including <br /> records for universal waste disposal (fluorescent bulbs, alkaline batteries, electronics, etc.). <br /> Provided: Modified contingency plan template, Sample label, Hazardous waste labels, Training schedule <br /> of free classes <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($116). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Ins Or Received By: Title: <br /> EHD -00 Rev 06/01/10 CONTINUATION FORM <br />
