Should you <br />have any questions or concerns please do not hesitate to call. <br />Line Item 108: We have taken samples of the waste in question and have had them analyzed by <br />Test America to determine if the waste is hazardous. The STLC for the waste <br />was above the limit for lead. We will be disposing the waste as a hazardous <br />waste. <br />Line item 1 17: Even though all our employees received required training we were <br />incomplete in documenting what training was required. Please find <br />attached document 400-0006-000, fulfilling the requirement. <br />Line item 122-124: See attached signed copies of manifest 008483834JJK and <br />008483833JJK and written explanation for manifest 009479250JJK <br />Line item 812: We have created a log and procedure to keep track of Universal Waste <br />generated at our facility.
