• EXHIBIT IV • <br /> FACTORY INSTRUCTIONS FOR MANAGING PCB LEAKS <br /> In the event that a PCB leak should occur, action shall be taken to insure <br /> that there is no contamination to the food product; nor runoff in waste <br /> water systems where PCB could re-enter the food chain cycle. Only those <br /> employees who have been trained and designated as "PCB Spill Crew" <br /> members shall be assigned to PCB cleanup activities. The following <br /> instructions have been developed to maintain order and minimize losses <br /> due to such spills or leaks. <br /> 1 . REPORTING LEAK <br /> A. Notify local management staff <br /> 1 . Factory Manager or Assistant <br /> 2. Plant Engineer or Assistant. <br /> 3. S.S.S. Manager <br /> 4. Chief Chemist <br /> 5. Chief Electrician <br /> B. Notify the following division management personnel immediately <br /> 1 . Vice-President of Operations <br /> 2. Head Maintenance Engineer or S.S.S. Coordinator, who will <br /> notify: <br /> a. Law Department <br /> b. Corporate Q.A.D. <br /> C. Applicable State or Federal Agencies <br /> II . CONTAINMENT <br /> A. Alert members of the PCB spill crew to respond <br /> B. In the event of product contamination: <br /> 1 . Divert and isolate the product <br /> 2. Determine if any machinery, conveyors or tankage <br /> surfaces which come into contact with the food <br /> product have been contaminated <br /> a. If not contaminated,resume operations when the <br /> leak is controlled <br /> b. If contaminated, shut down operations and <br /> decontaminate the affected surfaces <br /> C. Barricade the contaminated site and keep all unauthorized <br /> personnel out of the area <br />
