Thornton Rd. Lodi, CA 95242 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />spill control absorbent next to the 1 OK diesel gallon tank. Facilities shall be maintained and operated to <br />minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water <br />which could threaten human health or the environment. Immediately , and manage according to Title 22 <br />hazardous waste regulations. Submit a statement and supporting documentation explaining how this <br />waste was managed by 4-29-12. <br />605. Two 55 gallon drums of hazardous waste absorbent next to the water tank and one 55 gallon drum <br />of hazardous waste absorbent next to the mini mart lacked accumulation start dates. Nick Mostafavi <br />stated that the hazardous waste hauler just delivered the drums yesterday. All hazardous waste <br />containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2, name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />During the inspection Nick Mostafavi added the accumulation start date of 3-28-12. Continue to ensure <br />that all hazardous waste containers are marked with all the required information. Corrected on site. <br />812.. On 3-9-I, twenty-seven universal waste (UW) lamps were disposed of at SJC Lovelace MRF/ <br />Transfer Station. Site mechanic Francisco Avila confirmed that he had disposed of the UW lamps and <br />gave the receipt to Nick Mostafavi. Nick confirmed that he had faxed a copy of the UW lamp disposal <br />receipt to the EHD but cannot locate them at this time. A UW handler shall keep a record of each <br />shipment of UW sent from the UW handler's facility to another facility. The record may take the form of a <br />log, invoice, manifest, bill of lading or other shipping document. The record for each shipment of UW <br />sent shall include the following information: <br />(1) The name and address of the UW handler or destination facility to which the UW was sent; <br />(2) The quantity [count or weight, consistent with, for example, section 66273.32, subsection (d)] of each <br />type of UW sent (e.g., batteries, thermostats, lamps, electronic devices, CRTs, CRT glass); <br />(3) The date of departure of the shipment of UW. <br />Correct immediately by maintaining a copy of UW disposal records for three years. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 4-29-12. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Aris Cacapit (209) 468-3453/��C <br />Received By: <br />�1 .
