Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By 4-14-11, submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception <br />report submitted to DTSC. 60 Class II - Repeat Violation. 61 & 62 Minor - Notice to Comply. <br />79. In the truck lube building, approximately ten universal waste lamps were stacked against the <br />southeast corner of the building and the box of universal waste lamps was left open. A universal waste <br />handler shall manage lamps in a way that prevents breakage or release to the environment. Correct <br />immediately by properly containerizing the universal waste lamps. Class II - Repeat Violation. <br />83. The universal waste lamps at the southeast corner of the truck lube were not labeled as universal <br />waste lamps. Correct immediately by marking the universal waste lamps as universal waste lamps. <br />Class II - Repeat Violation. <br />84. The universal waste lamps at the southeast corner of the truck lube were marked with an <br />accumulation start date of 3-2-09. The universal waste lamp outside of the box stacked at the southeast <br />corner of the truck lube did not have documentation demonstrating how long they have been a waste. A <br />universal waste handler shall accumulate universal waste for no longer than one year and must <br />demonstrate when the universal waste became a waste. Correct immediately by properly disposing of <br />the universal waste and submit a copy of the disposal record to this office by 4-14-11. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 4-14-11. <br />This facility generated 30 tons of hazardous waste in 2010. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Ins <br />n� <br />Reoeived By: <br />Title: <br />EHD 23-02-003 Rev 08%10110 CONTINUATION FORM <br />
