Valpico Road, Tracy CA Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS 11 or MINOR-Notice to Cpm <br /> #49 Personnel training records incomplete: <br /> At time of inspection facility personnel training is incomplete. At a minimum,the training program shall be designed to ensure <br /> that facility personnel are able to respond effectively to emergencies by familiarizing them with emergency procedures, <br /> emergency equipment,and emergency systems, including where applicable, <br /> 1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-water contamination incidents; and <br /> 6. shutdown of operations. <br /> Facility personnel shall take part in an annual review of the initial training received. The owner/operator shall maintain the <br /> following documents and records at the facility: <br /> a)the job title for each position at the facility related to hazardous waste management, and the name of each employee filling <br /> each job; <br /> b)written job description for each position listed in a)above; <br /> c)written description of the type and amount of both introductory and continuing training that will be given to each person <br /> Ming positions listed in a)above; <br /> d)records that document that the training has been given to and completed by facility personnel. <br /> 7rdning records on current personnel shall be kept until closure of the facility. Training records on former employees shall be <br /> kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial <br /> or refresher hazardous waste training. Within 30 days(July 10,2010)provide this office with a copy of the roster and the <br /> syllabus which meet the requirements of 88265.16 CCR(California Code of Regulations). <br /> 060 Failed to keep signed copy for 3 years: <br /> 063 Failed to keep a copy of the consolidated manifest for 3 years: <br /> #23/64 Failed to retain a copy of manifest or bill lading for used oil filters/spent lead acid batteries: <br /> #85 Failed to keep records of shipment for each shipment of universal waste: <br /> At time of inspection facility could not produce any disposal documentation for hazardous waste. Facilities must maintain <br /> records for a minimum of 3 years. Within 30 days, July j�2010, provide this office with copies of all disposal records for the <br /> past 3 years. Begin maintaining disposal records. <br /> #79 UW lamps storage: <br /> At time of inspection several boxes, > 15 boxes,of used lamps were seen stored in open containers. Several lamps were <br /> seen freestanding. 4 broken lamps were seen with duct tape on one end. Broken pieces of lamps were seen in a plastic <br /> yellow container. Facilities must store UW lamps in a closed container and in a manner that prevents breakage and releases <br /> to the environment and immediately clean and containerize and broken lamps or releases to the environment. Immediately <br /> properly store UW lamps. Immediately clean and containerize the broken lamps. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE(;115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD I or.
