Minor Violations: <br />Notes: <br />• Approximately 10 used lead acid batteries are stored onsite and some with white dried powder on <br />top surface. After disposal to a hazardous waste disposal center, retain the disposal receipts <br />onsite for a minimum of 3 years. <br />• 4 burnt universal waste fluorescent tubes still inside the ballast located inside the shop. When <br />ready for transportation, fluorescent tubes must be in a secure container and properly identified <br />with the following information: words "Universal Waste fluorescent lights". After disposals to a <br />hazardous waste disposal center, retain the disposal receipt onsite for a minimum of 3 years. <br />• Provided hazardous waste instruction for labeling containers <br />• Provided free SJC EHD 2010 training schedule <br />• In year 2009, facility generated approximately 220 gallons of used oil. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($722). <br />THIS FACILITY IS UBJECT TO REINSPECTION AT IME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: THUY TRAN <br />Received B . <br />Titl AA <br />
