Walnut Grove Rd., Thornton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Com I <br /> of inspection, it could not be demonstrated (not necessarily documented) that <br /> employees who handle hazardous waste were properly trained. According to Mr. Tarin, the employees <br /> do not receive any additional training regarding hazardous waste besides what is covered during the <br /> UST designated operator training. The generator must ensure that all employees who handle <br /> hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. <br /> Immediately provide training to all employees who handle hazardous waste and submit a copy of the <br /> training records to the EHD by May 22, 2014. <br /> Notice to Comply - Minor violations: (122, 125, 128, 605) <br /> 122 & 125. Copies of hazardous waste disposal records for the last three years were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br /> receipts used in a consolidated manifesting procedure on site for three years and have them readily <br /> available for review. Immediately locate a copy of all manifests and receipts for the last three years, <br /> maintain them on site, and submit copies to the EHD by May 22, 2014. <br /> 128. Bills of lading for the recycling of used filters for the last three years were not found on site. A <br /> copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br /> bills of lading for used filters for the last three years, maintain them on site, and submit copies to the <br /> EHD by May 22, 2014. <br /> 605. A 30 gallon black steel drum of hazardous waste liquid was observed in the dumpster area with a <br /> faded, illegible hazardous waste label. A gray plastic container of fuel contaminated solids (hoses, <br /> nozzles, filters, absorbent) mixed with trash was observed without a label behind the store; according to <br /> Mr. Tarin, this is the hazardous waste solids storage container. All hazardous waste containers shall be <br /> marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: <br /> Received By: Title: <br /> Stacy Rivera (209) 468-3440 A/ <br /> CONTINUATION FORM <br /> EHD 23-02-003 Rev 04/19/12 <br />
