The following is Class II Repeat violation: 101, and <br />Minor -Notice to Comply: 407 <br />101. This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, <br />dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. <br />Immediately contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by <br />05/30/2013. A form has been provided. <br />407. Observed one 55 -gallon empty steel drum with a hazardous waste label under parts of a removed <br />cyclone fence. Containers larger than five gallons that previously held a hazardous material shall be <br />marked with the date they were emptied and managed within one year of being emptied. Immediately <br />mark and manage all empty containers per this section. <br />NOTES <br />Facility is a retail gas station and generates hazardous waste from customer spillage or monitoring <br />system certification. No hazardous waste was observed or had been generated in 2012 during this <br />inspection. Discussed universal waste management, and supplied a copy of San Joaquin County Small <br />Business Hazardous Waste Disposal Program pamphlet. Submit the following to EHD by: 05/30/2013 <br />1) A completed copy of the (attached) Return to Compliance Certification (please keep original for <br />your records). <br />2) A written statement how each violation was corrected <br />3 Evidence i.e. photos, disposal receipts, documents in support of the corrections made. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />inspector: Received By: r i JTitle <br />M : <br />EHD 22-02 Rev 0411902 _/ Page 4 HW INSPECTION REPORT <br />
