N � <br /> The following is an itemized list of hazardous waste violations that have not been <br /> addressed for PRESTIGE AUTO BODY & PAINT as of December 24, 2015. <br /> Open violations from July 08, 2015 inspection <br /> Violation#101 -Managed a hazardous waste without a valid EPA ID number. <br /> This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or <br /> offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and reactivate you[ <br /> EPA ID number and submit evidence to the EHD. A form has been provided. <br /> Violation#109 -Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for July 2012 through July 2015 were not found on <br /> site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipt <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> Violation#112-Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for July 2012 through July 2015 were not found on site. Hazardous was <br /> generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidate <br /> manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy <br /> all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> Violation#117-Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Violation#403 -Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> Brake shavings found inside two metal grinders on the north side of the facility were not containerized . All hazardou <br /> waste containers shall be closed at all times except when adding or removing waste. Immediately close these <br /> containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Violation#605 -Failed to completely label containers or tanks of hazardous waste. <br /> Brake shavings found inside two metal grinders on the north side of the facility were not labeled . All hazardous wase <br /> containers shall be marked with the following information: <br /> -the words"Hazardous Waste' <br /> -name and address of generator <br /> - hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required information. Since <br /> these brake shavings have been on site longer than 180 days, immediately contact a licensed hazardous waste haul <br /> to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> Page 1 of 1 <br />
