California 1506/Store Claim Environmental Self Audit On a weekly basis,use the following checklist to conduct a Hazardous Waste self- inspection at your store. Performing the tasks listed below will help you ensure that. 1506/store claim procedures are being properly executed and are in compliance with the 1506/Store Claim program. Note: This form is to be retained in the office file cabineton a rolling weekly basis. Be sure that as the new form is filed each week,and that the previous form is discarded. Daily Tasks: ❑ Verify that all unsalable 1506/store claims are being completed each day. ❑ Verify that all totes have lids firmly in place. ❑ Verify each tote has a label,and that the label has been completed properly. . ❑ Verify there Is both a P-Rated and RX tote with a lid in the pharmacy. Weekly Tasks: Self-Serve Items for Evaluation,Totes: ❑ Confirm that each tote with 1506/store claim product within it has a lid firmly in place. ❑ Open several totes and visually Inspect several of the zip seal bags(do not unseal or open the bags): o Confirm each zip seal bag contains oniythe same item/same UPC. o Check for spill kit absorbent in the zip seat bag of the item(s)that once contained liquid. o If an item(s)was damaged,ensure that the item in the zip seal bag was placed in an additional zip seal bag(total of two sealed bags). ❑ Ensure there is no free flowing liquid in the bottom of the tote. ❑ Verify there is a label on each tote that has within it 1506/store daiin product waiting for evaluation. J) ❑ Confirm if the date on the label is appropriate,considering the last pick up date by the 1 environmental waste hauler.(i.e.If the label is dated 12/2010,but the last pick up at the store was 1/2011 this is not appropriate,product should have already been picked up in January), ' Photo Spill Kit(if Applicable): ❑ Confirm the cover has been placed securely on th'e container and turned clockwise. ❑ Verify the photo spill kit has a"Corrosive" label placed on the outside of the bucket/container. ❑ Verify that both labels(Hazardous Waste and Cautionary label)have been placed on the outside of the Spill Defense Kit'container. Rx items for Evaluation,Tates: ❑ Verify the Rx Tote(for items needing further evaluation)and the P-Rated stock bottle l totes both have lids firmly in'place. j ❑ Open the Rx Tote,and verify that the items within the tote have been placed in a zip seal bag:
