(Attachment 1). During the <br /> inspections, Mr. von Flue verbally notified Mr. Chacko Thomas that this facility does not have a <br /> permit to operate the underground storage tanks. Mr. von Flue also informed Mr. Thomas that a <br /> permit would not be issued until the results of the secondary containment testing were received <br /> by EHD, all fees were paid in full, and documentation requested from the routine underground <br /> storage tank inspection were received. Mr. Thomas acknowledged this and also stated that the <br /> ownership of the facility would be changing in a few months. The listed owner of the <br /> underground storage tank at this facility is Robert Lease, 2307 Yosemite Ave., Escalon, CA. Mr. <br /> von Flue informed Mr. Thomas that State A and B forms would need to be completed by the new <br /> owner and to contact Mr. von Flue to acquire the forms. Mr. von Flue found minor violations on <br /> the hazardous waste inspection. <br /> On September 30, 2002, letters were mailed to the operator, tank owner and business reminding <br /> them of the secondary containment-testing deadline(Attachment 2). <br /> On November 15, 2002, certified letters were mailed to the operator, tank owner and business <br /> giving final notification for testing the secondary containment by December 31, 2002 <br /> (Attachment 3). <br /> On January 17, 2003, A and B forms were submitted to change the status to non-compliance; <br /> therefore, a permit to operate was not issued(Attachment 4). <br /> On February 3, 2003, Mr. von Flue went to the facility to document if the underground storage <br /> tanks were still operating without a permit, and found the facility was still operating.
