03 <br />Page 2 <br />* A business is required to report any release or threatened release of <br />hazardous material to these agencies if it poses a serious threat to the <br />environment or human life. <br />B. If a spill or release occurs where no outside emergency <br />response assistance is needed and where a CERCLA <br />reportable quantity is only suspected, the incident is <br />to be reported to the Spill Coordinator as soon as <br />Possible. They will then help determine if a <br />reportable quantity was released and that all <br />appropriate agencies have been notified. <br />C. Whenever any emergency response agency is called to <br />report a spill or release, an "Incident Report" is to <br />be completed, explaining the incident, what happened <br />during the call, and sent to the Manager, Environmental <br />Protection in S.F.O. <br />D. If the decision has been made to contact outside <br />agencies for assistance or for reporting requirements, <br />the following information should be gathered. <br />1. Location of occurrence. <br />A. If in a patient area, is evacuation needed? <br />2. Determine if there are injuries. <br />A. Extent of injuries. <br />3.. Name, address, phone number of reporting party. <br />4. <br />Type <br />of incident: spill, leak, fire, vapors, <br />vehicle accident, dumping, etc. <br />A. <br />If a fire is involved, how long has <br />it been burning? <br />B. <br />Type of containers (pressurized, <br />etc.) <br />C. <br />If leakage or spilled load, how <br />long has substance been leaking? <br />D. <br />Quantity of material. <br />5. <br />Name <br />of material and hazard class: <br />A. <br />Hazard Classes: <br />1. Explosive 5. Oxidizer <br />2. Gases 6. Poison <br />3. Flammable Liquid 7. Radioactive <br />4. Flammable Solid S. Corrosive <br />6. <br />Weather Status <br />A. <br />Wind speed and direction <br />7. <br />Environmental threat <br />A. <br />Water, soil, air. <br />
