All analytical results must come directly from a third party California certified <br /> laboratory with affixed chain of custody or this office will not accept them. <br /> 2. All hazardous waste must be treated, stored or disposed of at a permitted facility. <br /> 3. See pages 7 & 8 of the permit application for required analytical tests. <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. In addition,the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) Analytical results,mailed from the lab <br /> (b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> (c) Tank Closure Certification Form (DTSC form 1249) <br /> (d) Hazardous Waste Manifest for piping, rinsate,residual fuel, or waste oil receipt <br /> 6. This permit expires on February 2, 2004. <br /> 7. This site shall be sampled for unknown fuel. <br /> 8. The numbers of samples to be taken will be in accordance to the tank size as per page <br /> 7 of this permit. <br /> 9.
