PUBLIC HEALTH SERVICES <br /> SAN JOAQUIN COUNTY �' z <br /> ENVIRONMENTAL HEALTH DIVISION ` <br /> Karen Furst, M.D., M.P.H., Health OfficerP. <br /> 304 East Weber Avenue, Third Floor • Stockton, CA 95202 <br /> 209/468-3420 <br /> April 23, 1998 <br /> AftnEdRalston <br /> 200 Cr <br /> 2000 Crow Canyon Place, Ste. 400 <br /> San Ramon CA 94583 <br /> RE: Circle K #1205 SITE CODE: <br /> 16470 Cambridge Ave. <br /> Lathrop CA 95330 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) <br /> has reviewed the analytical results of the soil and/or water samples collected beneath <br /> the underground storage tank(s) that was/were permanently closed at this site on <br /> November 11, 1997. <br /> Based on the levels of contaminants present in the samples collected, further <br /> investigation to determine the extent of the soil and/or groundwater contamination is <br /> required. <br /> An "Underground Storage Tank Unauthorized Release (Leak)/Contaminated Site <br /> Report" has been filed pursuant to Section 25295 of the Health and Safety Code <br /> (HSC). The appropriate disclosure of a hazardous waste discharge pursuant to Section <br /> 25180.7 of the HSC has been made, and the site has been placed on the list of <br /> contaminated sites in San Joaquin County which is maintained by PHS/EHD. <br /> PHS/EHD under contract with the State of California Water Resources Control Board <br /> (SWRCB) oversees the investigation and remediation of contamination from leaking <br /> UST's. Responsible parties must take corrective action in response to an unauthorized <br /> release, in compliance with Section 25299.37 of the HSC and Corrective Action <br /> Regulations adopted pursuant to Section 25299.77 of the HSC. <br /> PHS/EHD uses the SWRCB "Prioritization System for Leaking Underground Storage <br /> Tank Sites" to prioritize all the contaminated sites. You will be directed to begin the <br /> required investigation, based upon the priority ranking of the site and PHS/EHD staff <br /> availability. <br /> A Division of San Joaquin County Health Care Services <br />
