HEALTH and SAFETY PLAN BROOKS PRODUCTS, INC. 41 E C E I V SEP 201900 ENVIRONMENTAL HEALTH PERMIT/SERVI(rES The site location for the proposed job or removing one (1) 2,000 gallon under- ground gasoline tank is as follows: Brooks Products, Inc 2441 Charter Way Stockton, Ca. (209) 464-7696 The project manager, and the health and safety coordinator is Wesley W. DuBose of Western Meter Service, Inc., 2735 Teepee Drive, Suite E, Stockton, Ca 95204, (209) 948-6124. The site safety officers at the job site will be Wes- ley W. DuBose and/or Dan Ensley of Western Meter Service, Inc. The alternate site safety officer will be John Swets of Western Meter Service, Inc. Western Meter Service, Inc. will have on the site two (2) fire extinguishers and one (1) first-aid kit within 50' of the tank location. See attached map. No smoking will be permitted within 50' of the tank or areas where flammable vapors are present. Western Meter Service, Inc. shall notify the local Fire Department 48 hours before excavation shall begin. USA will be notified before excavation of job. Tanks must be empty of product before start of job. Western Meter Service, Inc. will triple rinse tanks with soap and water. Rinse will be hauled away by Refineries Service, Inc., Patterson, Ca 95363, (209) 892-6742, a waste haz- ardous hauler. Tanks will be dry iced, 30 pounds of dry ice per 1,000 gal- lons, before excavating. Western Meter Service, Inc. shall monitor tanks with a Combustible Gas Indicator (CGI) and seal all openings before tanks will be transported. At no time will Western Meter Service, Inc. cut or weld on the tank. Tanks will be secured on truck after extraction for transportation. Tanks and plumbing will be hauled by Western Meter Service, Inc., 2735 Teepee Drive, Suite E, Stockton, Ca 95205, (209) 948-6124 to Triangle Inc. of Sacramento, 3525 52nd Avenue, Sacramento, Ca 95823, (916) 421-1990 to be destroyed. All of the above procedures will be done according to code. Western Meter Service, Inc. will approach all tank excavation with possible cave-ins. The following rules will be followed to prevent any mishaps: 1) soil to be inspect for stability before and after tank is removed, 2) soil is to be keep at least two (2) feet from the edge of an excavation, 3) if excava- tion is five (5) feet or more the hole shall be tapered, braced, or other equivalent methods shall be used to prevent possible cave-ins.
