Cherokee Lane <br /> Lodi, CA <br /> On April 22, 1994, San Joaquin County Public Health Services, Environmental <br /> Health Division (PHS/EHD) staff witnessed the removal of piping associated with <br /> the underground storage tanks that were formerly located at the above referenced <br /> site. Soil sample results and observations of the PHS/EHD staff confirm that <br /> petroleum hydrocarbons have contaminated the soil in the area of the dispensers <br /> closest to Lodi Avenue. <br /> In a letter dated February 3, 1994, PHS/EHD notified you that the laboratory <br /> method used to analyze the soil samples collected beneath the tanks in 1988 was <br /> inappropriate and the number of samples collected were insufficient. <br /> On June 6, 1994 State Water Resouces control Board (SWRCB) staff with the <br /> Underground Storage Tank (DST) Cleanup Fund Program (Cleanup Fund) reviewed the <br /> UST file of PHS/EHD to determine if you are in compliance with corrective action . <br /> orders and directives for this site so a Letter of Commitment from the Cleanup <br /> Fund can be issued. <br /> PHS/EHD is directing you to submit a work plan to investigate the extent of the <br /> contamination at this site by August 1, 1994. If you have not submitted a work <br /> plan by that date we will notify SWRCB that you are not in compliance with our <br /> directives and your eligibility for cleanup funds will be jeopardized. <br /> Please find enclosed a list of consultants that can be contacted for work plan <br /> preparation.
