The facilities Permit to Operate <br /> will be issued for a. five year period. In addition to the yearly <br /> inspectional fee, this statement will also include a #56/tank State surcharge <br /> fee. TKY Stvis surcharge fee per tank will be charged every five years or <br /> whenever the facilities permit is renewed or arTnd9d.
