Cluff Ave. Lodi. <br /> I have reviewed the above referenced application. Please submit the following <br /> information to this office. <br /> 1. You need to complete the enclosed service request form. <br /> 2. The contractor performing the removal cannot sample. Please identify a third <br /> party sampling firm. <br /> 3. You need to complete page 4, #7, e. <br /> 4. You need to complete page 5, #8, b & c. <br /> 5. The dry ice requirement for inerting the tanks is 30lbs/1000 gallon capacity. <br /> 6. If Lodi Unified School District is responsible for additional PHS-EHD staff time <br /> beyond the initial permit payment, then they must acknowledge this <br /> responsibility by signature and date on page six. <br /> 7. Cal Coast Dairy System is not an acceptable disposal facility for the tanks and <br /> piping.
