CLUFF AVE,LODI <br /> EQUIPMENT DESCRIPTION: <br /> ONE(1) 10.000 GALLON GASOLINE STORAGE TANX SERVED BY OPW COAXIAL PHASE I VAPOR RECOVERY SYSTEM <br /> AND ONE (1)NOZZLE SERVED BY BALANCE PHASE 11 VAPOR RECOVERY SYSTEM. <br /> CONDITIONS <br /> 1. All nozzles shall be equipped with coaxial hose configurations. <br /> 2. At least 95% by weight of all gasoline vapors displaced during the filling of storage tanks and the <br /> refueling of vehicles shall be prevented from entering the atmosphere. <br /> 3. The vapor recovery system and its components shall be operated and maintained in accordance with the <br /> State certification requirements. <br /> 4. The permittee shall perform a Dynamic Back Pressure Test using CARE Method TP-201.4 within 60 days after <br /> initial start-up and at least once every five years thereafter. <br /> S. The permittee shall perform a Leak Test using CARB Method TP-201.3 within 60 days after initial start-up <br /> and at least once every five years thereafter. <br /> 6. The District shall be notified by the permittee 15 days prior to each test. The test results shall be <br /> submitted to the District no later than 30 days after each test. <br /> 7. Prior to July 1, 1994, and at least once every five years thereafter. each vapor recovery system shall <br /> be tested to determine proper-installation-and function using District approved test methods. <br /> This Permit to Operate remains valid through the permit expiration date listed above, subject to payment of annual permit fees and <br /> complianoa with permit conditions and all applicable local, state, and federal regulations. This permit is valid only at the location <br /> specified above, and becomes void upon any transfer of ownership or location. Any modification of the equipment or operation. as <br /> defined in District Rule 2201, will require a new permit. This permit shall be posted as prescribed in District Rule 2010. <br /> DAVID L.
