The following is an itemized list of underground storage tank violations that have not <br /> been addressed for ARCO amlpm # 83230 as of April 03, 2017. <br /> Open violations from October 22, 2015 inspection <br /> Violation #314 -Secondary containment not tight. <br /> 1) The spill and vapor container shroud boots and band clamps were missing on the 87 and 91 spill containers, <br /> vapor opening cap was missing on the diesel fill sump lid. Secondary containment shall be impervious to the liquid <br /> and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with <br /> any hazardous substances released from the primary containment. Immediately contact a properly licensed, <br /> trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD. THIS <br /> IS A REPEAT VIOLATION CITED DURING THE2013 AND 2014 INSPECTIONS, <br /> 2)An accumulation of the interstitial fluid (1-2 cups) was noted at the bottom of the diesel fill sump. The technician <br /> onsite indicated that sump's brine is leaking from the secondary containment, in order to verify the leak, he <br /> immersed a white towel into the accumulated liquid, when removed, the towel took on a bluish tint indicating a leak <br /> in the secondary containment. Secondary containment shall be impervious to the liquid and vapor of the substance <br /> contained and constructed to prevent structural weakening as a result of contact with any hazardous substances <br /> released from the primary containment. Immediately contact a properly licensed, trained, and certified contractor to <br /> make repairs to the UST system under permit and inspection of the EHD. <br /> 3) During the inspection it was noted that holes were drilled in the collar of the 91-octane fill sump and sump lid <br /> These holes were drilled to install the sump lid clamps. The liquid residue trails (white in color) were noted <br /> originating from the areas where the holes were drilled and bolts were installed. Secondary containment shall be <br /> impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a <br /> result of contact with any hazardous substances released from the primary containment Immediately contact a <br /> properly licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of <br /> the EHD. <br /> Violation #316 -Secondary containment not constructed to prevent water intrusion. <br /> Water was found in the diesel STP sump(approximately 114 cup), 91-octane STP sump (approximately 1 oz.), <br /> 87-octane STP sump (approximately 1 oz.), 91-octane fill sump (approximately Y2-cup), 87-octane fill sump <br /> (approximately 1 quart), and UDC 13114 (approximately 1 oz). Secondary containment shall be constructed to <br /> prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. The service technician <br /> removed all the noted liquid. Immediately contact a properly licensed, trained, and certified contractor to address the <br /> water intrusion into all above sumps under permit and inspection of the EHD. <br /> Open violations from October 29, 2014 inspection <br /> Violation #314 -Secondary containment not tight. <br /> The spill container shroud boots (on all USTs)were either cut or were missing metal clamps to hold shroud boots in <br /> place. This is a repeat violation which was sighted on the October 28, 2013 inspection report. Secondary <br /> containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent <br /> structural weakening as a result of contact with any hazardous substances released from the primary containment. <br /> The permit for these repairs (SR0070287) was obtained from the EHD, immediately contact a properly licensed, <br /> trained, and certified contractor and make repairs to the UST system. This is a repeat violation, Class 11 <br /> Violation #316 -Secondary containment not constructed to prevent water intrusion. <br /> Water was found in the diesel STP and diesel fill sumps and in UDC 13114. Secondary containment shall be <br /> constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. The service <br /> technician removed less than 1 cup of liquid from each aforementioned sump/UDC and returned the sensor to its <br /> proper location. Immediately contact a properly licensed, trained, and certified contractor to address the water <br /> intrusion into the sump under permit and inspection of the EHD This is a repeat violation, Class II. <br /> Open violations from October 28, 2013 inspection <br /> Shroud boots are cut and sump lids are lacking clamp-down latches <br /> Page 2 of 2 <br />
