The designated operator failed to respond with follow up action for the L31 <br /> 87-annular low liquid alarm on 3/31/16. The designated operator failed to complete Items 1 through 4 on the <br /> 9/28/16 monthly inspection report. During the monthly inspection, the designated operator shall review the alarm <br /> history for the previous month, check that each alarm was documented and responded to appropriately, and attach <br /> a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> This is a minor violation. <br /> 304 HSC 25290.1(d) Monitoring system not maintained to detect entry into secondary containment(after Jul 2004). <br /> The 87-octane fill sump 208 sensor was found leaning over and not positioned to detect a leak at the earliest <br /> opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br /> opportunity. The service technician placed the 87-octane fill sump 208 sensor in an upright position after completing <br /> the testing. <br /> This was corrected on site) <br /> This is a minor violation. <br /> 305 HSC 25290.1(e)VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> Vacuum sensor monitoring of the secondary containment piping of the vacuum/pressure/hydrostatic(VPH) system <br /> has not been tested to date, nor has communication in the three submersible turbine pump (STP)sumps and three <br /> fill sumps been tested. The interstitial space of the underground storage tank shall be maintained, as designed, <br /> under constant vacuum such that a breach in the primary or secondary containment is detected before the <br /> hazardous substance is released into the environment. Immediately contact a properly licensed., trained, and <br /> certified contractor to test communication of the VPH components under inspection of the Environmental Health <br /> Department. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 87-octane fill sump 208 sensor was found leaning over and not positioned to detect a leak at the earliest <br /> opportunity.
