Box 2009 • (1601 East Hazelton Avenue) • Stockton, California 95201 (209) 468-3400 TO: INSTALLERS AND OWNERS OF NEW UNDERGROUND STORAGE TANKS San Joaquin County Public Health Services -Environmental Health Division (PHS-EHD) has established the following guidelines to assist you in the completion of your construction project. The guidelines specify the construction and monitoring criteria necessary to complete our review of your proposed underground storage tank (UST) system. 1. SUBMITTAL OF PLANS a. Submit 3 complete sets of detailed drawings in non -erasable print to the Environmental Health Division. Upon approval, one set will be retained by the department and the other two will be returned. b. Include equipment and material specification sheets and manufacturer's brochures describing operating and monitoring systems and listing numbers or other evidence of a nationally recognized testing organization approval. / c) Include detailed monitoring and response plans for facility. The drawn to scale plot plan should illustrate the location of all existing and proposed ✓ structures, including existing UST(s). e. The plans should include a cross sectional diagram of the tank and piping system including secondary containment, overflow protection equipment, and extension of all pipes and/or ports from subgrade to finish grade. 2. PLAN REVIEW AND OPERATING PERMIT FEES a. New facility plan review fee of $624.00 (8 hour minimum per facility), $170.00 fee per tank, plus $56.00 state surcharge fee per tank is due at time of plan submittal. b. New installation of additional tank at an existing facility is $624.00 plan review fee (8 hour minimum per facility) plus $170.00 fee per new tank, and 556.00 per tank state surcharge fee. C. It should be noted that a $624.00 plan review check fee required for each submittal allows for up to eight (8) hours of staff review and/or inspection time. Additional time spent on plan review, meetings, inspections, phone or office consultations will be billed to the applicant at a rate of $ 78.00 per hour. d. The maximum review time for UST installation plans is 40 working days. A preliminary review will be conducted by the PHS/EHD within 20 working days of submittal date to identify any gross plan deficiencies. e. If gross deficiencies are identified, the plans will be returned with checklist identifying plan deficiencies. f To expedite the review of installation plans, time and a half per hour overtime fee may be submitted to have review completed within 10 working days of submittal date.
