D-104 <br />Kent, Wa 98032 <br />This letter is in response to our conversation on 8-12-97 <br />regarding leak detector and probe replacement and the <br />requirements for repair permits. The facility where the repairs <br />were made is located at 2725 Country Club Blvd. Stockton, CA. <br />PHS-EHD uses Title 23 CCR Section 2711 and the Health and Safety <br />Code to require repair permits, for changes, to the monitoring <br />system. The Health and Safety Code Section numbers are 25286 (a) <br />and 25299.2 (copies attached). PHS-EHD has developed a policy in <br />regards to this issue (copy provided). The reason for the repair <br />permit is to assure that the devices are State approved (LG113- <br />11) and that they are properly installed and functioning. In <br />order to resolve this issue you must take the necessary steps to <br />obtain a repair permit.
