The permit has been <br />approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact. Muniappa Naidu at (209) 468-3430 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of pern-IJ <br />approval. <br />a. Overfill prevention verification: Measurement of drop tubes to verify 95% tank volume <br />shut off. <br />b. Corrosion protection: Verify corrosion protection for any component in direct contact <br />with the backfill material prior to backfilling. <br />c. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />4. Since concrete will be broken near the UST, a secondary containment test must be <br />performed on the UST once concrete is poured in accordance with the manufacturer's <br />guidelines. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a.
