Cuellar has been designated to coordinate access control and security onsite. A safe
perimeter has been established at 20 feet around boring/excavation. No unauthorized person
should be within this exclusion area.
4.0. HAZARD EVALUATION
The following substance(s) or hazards are known or suspected to be on site.
SUBSTANCES INVOLVED: CONCENTRATIONS:
Heating oil (?) unknown
Other organic-based products (?) I unknown
The primary hazards of each are identified as follows:
1. Hydrocarbon fuels- Components of fuels are colorless liquids with strong aromatic petroleum
hydrocarbon-like odors. They are flammable and have low solubility in water. Exposure to
these materials in excess of recommended exposure limits can cause mild eye, nose, and
throat irritation and can produce a narcotic effect on the central nervous system. Symptoms
of exposure at these concentrations include headache, nausea, drowsiness, dizziness, and loss
of coordination.
The following additional hazards are expected on site: typical work related to facility. Noise falling
rods flying hammer snapped cables etc on the backhoe/excavator (if applicable).
5.0. PERSONAL PROTECTIVE EQUIPMENT
Based on evaluation of potential hazards, the following levels of personal protection have been
designated for the applicable work areas or task:
LOCATION: JOB FUNCTION: LEVELS OF PROTECTION:
Exclusion Zone Drill borings A B C D Other
Reduction Zone Preserve samples A B C D Other
and log borings
Support Zone Support functions A B C D Other
