In addition, the following information must be submitted within 30 days of the removal <br /> date: <br /> (a) Hazardous Waste Manifest fon,piping,rinsate,residual fuel, or waste oil receipt if <br /> applicable <br /> 5. All fill and access locations and piping shall be sealed using locking caps or concrete plugs_ <br /> 6- This permit expires on March 9, 23. <br /> 7.
