COSTCO.
October 2, 2015
Fatinah Zareef,
REHS
San Joaquin County EHD
1868 Hazelton Avenue,
Stockton, CA 95205
Dear Inspector Zareef
RECEIVED
NOV 0 2 26i5
ENVIRONMENTAL
Wr-61TW nCDA0TA4CK1Y
Thank you for your visit to our location on August 19, 215. During your visit, you brought a few items to our
attention. We have noted each issue below with our plan to address them.
Concern or Violation:
CCR 2632: Monitoring and response plans not current or approved by the EHD. The Leak sensor model
number is incorrect. The leak sensor model number is listed as 794380-301 for Tank ID #1, Tank ID #2,
and Tank ID 43. The correct leak sensor model is 794380-303 for all three tanks. The monitoring plan
must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS,
make the necessary changes, and submit for review by the EHD.
Response:.
The Monitoring and Response plan was uploaded to CERS on 9/14/15. The submittal was for revisions to existing
equipment — Leak Sensors. The Underground Storage Tank (UST) submittal was accepted and approved by Garrett
Backus, REHS, ICC Certified UST Inspector, San Joaquin County Environmental Health Department, on 9/15/15.
Concern or Violation:
CCR 2712: Failed to maintain alarm logs and/or records of follow up actions. The daily alarm logs did not
document all alarms from the alarm history. All alarms from the alarm history need to be documented in
the daily alarm log and kept on site for review by the EHD. Immediately begin documenting all alarms
from the alarm history in the daily alarm log and have available for review.
Response:
Effective immediately 08/19/15 the facility at 2440 Daniels Street is documenting all alarms from the Veeder-Root
to the daily alarm log; all documentation is currently available for review by EHD.
Concern or Violation:
• HSC 25290: VPH monitoring of the interstitial spaces of the of the UST system system is not maintained.
The 87A STP sump product S9 vacuum sensor failed when tested. The interstitial space of the UST shall be
maintained, as designed, under constant vacuum such that a breach in the primary or secondary
containment is detected before the hazardous substance is released into the environment
Response:
The sensor was replaced and retested during the inspection; issue corrected.
Please let me know if there is anything we may have overlooked, or if you would like any additional information.
We would like to be sure we have addressed all of your concerns.
