The following are <br />Class II Repeat violations: 305, 406 <br />305. The sensor in the 91 -octane fill sump was found to be defective when tested by technician as it <br />would not come out of sensor -out alarm (first discovered by contractor Elite IV, who reprogrammed it to <br />bypass positive shutdown on August 18, 2014.) All monitoring shall be maintained to activate an audible <br />and visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately <br />by having a properly licensed, trained, and certified contractor replace the failed component with a <br />functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from <br />the EHD. If the failed component can't be replaced immediately, there is a possibility that the UST <br />system may be red tagged to prevent fuel inputs. <br />The sensor was replaced on site by Alltech Petro, a licensed contractor company. Violation has been <br />corrected. <br />406. The Dispenser 9110 annular sensor failed when tested. The sensors monitoring the liquid level in <br />the interstitial space shall be maintained so that a breach in the primary or secondary containment is <br />detected before the hazardous substance is released into the environment. Correct immediately by <br />having a properly licensed, trained, and certified contractor replace the failed component with a <br />functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from <br />the EHD. If the failed component can't be replaced immediately, there is a possibility that the UST <br />system may be red tagged to prevent fuel inputs. <br />The sensor was replaced on site by Alltech Petro, a licensed contractor company.
